## The Vision We Are Pursuing

Standard medical AI tools summarize static PDFs or transcribe single visits. But real clinical medicine happens across multiple providers, hospital admissions, outpatient visits, and prescription adjustments over years.

When medical records are processed as isolated vector chunks, the AI is temporally blind:
- It repeats copy-forwarded "chart lore" (for example, an unconfirmed diagnosis entered in an urgent care note that was later refuted by a specialist test).
- It retrieves discontinued medications alongside active regimens.
- It fails to connect multi-drug prescribing cascades (where Drug A creates a side effect that triggers Drug B, causing complications that trigger Drug C).

CareNote is pioneering bi-temporal clinical memory: modeling patient health as an evolving knowledge graph with explicit validity boundaries and zero-blind-trust PaperTrail citations.

- **Your perspective:** You are the real-world frontline navigators who assemble fragmented records from multiple health systems into a coherent picture for families and physicians.

  - Pilot CareNote workspaces for your clients' multi-provider records.
  - Provide direct UX and workflow feedback on the PaperTrail audit inspector and timeline navigation.
  - Help shape patient-facing and family-facing longitudinal summary formats.

### 3. AI Engineers & Biomedical Graph Researchers
- **Your technical focus:** Context graphs, bi-temporal indexing, knowledge graph retrieval, multimodal document parsing, and verifiable citation mechanisms.
- **Your involvement:**
  - Collaborate on graph retrieval algorithms (combining entity nodes, chronological episodes, and temporal edges).
  - Benchmark extraction precision across multi-column laboratory reports, handwritten medication charts, and dense discharge summaries.
  - Build open-source clinical benchmarks and interoperability bridges.
