# CareNote: Longitudinal Clinical Memory & Verifiable PaperTrail

> Medical history is not a static text chunk. It is an evolving knowledge graph across time. CareNote turns fragmented clinical encounter PDFs into a persistent bi-temporal patient knowledge graph, producing longitudinal clinical syntheses where assertions can be inspected against source records through The PaperTrail.

## Core architectural thesis

Standard vector retrieval can lose temporal state across fragmented records. It may retrieve discontinued medications alongside active regimens, repeat copied-forward problem-list labels, and obscure relationships across encounters. CareNote models patient history as an evolving graph with explicit validity boundaries and source anchors.
